2011 Tax Return-How do I declare capital gains on shares e-tax?

-I acquired shares in Bank of Qld when they took over another institution I was part of a long time ago. I sold these shares this financial year for just over $2,000. e-tax requires that I enter a date of acquisition but I have no idea when I acquired it and I dont know how to find out :( Does anyone know how I might find this out or what I can put in there?



Also where it asks cost inquired to obtain asset do I put 0?
